Sciatica after surgery

I am 45 years old, about 9 years ago due to the rupture of the vertebral disk 4.5 I was carried out by Dr. Silkkar in Isfahan and after a few months ago I lived without any problems, but 4 months ago numerous clogging of the spine was created and after a while he was eliminated for 10 days.Pleasted to the left foot from under the groin to the calf so that at some time the fabric is not tolerated.I do exercises and use gabapentin and matharbags and only the dock relieves pain for a few hours..I have no pain or mild when lying down, but when standing after a minute of the needle's feet.The needle becomes the left and the left foot heel and the two fingers of the left toes are numb.please guide me
